After being selected as a monitor, please choose your preferred date and time from the available options.
*Earliest availability: Weekdays from mid-February 2026 onward.*
*Available appointment times (weekdays): Between 9:30 AM and 12:30 PM*

Approximately 60 minutes

**Flow from Application to Completion of the Survey Report**

1. Click the application button.
2. Submit your preferred appointment date and time via the dedicated form provided in the selection email.
*Please also provide your bank transfer details and confirm your consent.*
3. Once your reservation is confirmed:
You will receive a confirmation email. Please visit the dental clinic on your reserved date and time.
*If you receive a “rescheduling request” email, please select a new date and complete the reservation again.*
4. Complete the medical questionnaire (online).
5. On the day of the experience:
After the treatment, please pay the experience fee of ¥5,500 (tax included) at the reception desk.
**[IMPORTANT]** Be sure to receive a receipt. (Required to receive your reward.)
6. Submit your completion report (survey response and receipt submission).
7. After confirming the completion report, we will transfer the payment to your designated account. (Closing at the end of the month, payment on the 15th of the following month.)
